COT (cat over-temp) trigger?

I've read that cat overtemp is supposed to
"protect the cats from lean high temperature
conditions" or something like that. Which
makes me wonder if a well-tuned car will
ever have a reason to trigger COT. If the
engine never goes lean above 4500RPM, will
COT ever trigger? Is there a particular O2
volts threshold that, if you maintained it,
would keep you out of COT?
