Default Broken upper shock mount

Any ever had the center pull out of the upper shock mount? It only did it on the drivers side. I am hoping its just a bad mount. They were new when i put it all together as maybe 5k miles on them.

They will do that if you forgot to install the isolator (the flat wedge shaped thing that sits down inside the top of the mount).

I am sure this is the issue you have going on. If you don't have the strut plate insulator on the top of the strut mount to spread the load out it will pull the center right of the strut mount. Here is a picture of the piece we are talking about.
