Default When to change rotors/pads?

I had a set of Powerslot rotors and Durastop pads installed about 12k miles ago. The brakes now feel somewhat less than confidence inspiring. How can I determine what if anything needs replacing? The rotors look OK but I was wondering if there is some measurement I should go by to tell if the rotors and/or pads are worn out?
Thanks!

Rob

Usually you buy new pads when something feels funny. I would go with the Hawk pads..

having your brake fluid flushed/filled/bleed once every 6 months will work wonders

if you want to pull the calipers off and take a look at the pads... just look for 'even wear' on the pads.. they shouldn't be sloped(tapered)...
pulling the calipers is an easy job:
