Almost any place that does alignments will do a custom alignment.
I've had the dealership and Sullivan Tire both do my custom alignments.

I've paid between $60 and $80 for alignments. I think my last alignment a few months ago at Sullivan Tire was about $80 (4-wheel aligment to my specs).

(My car was re-aligned after an accident. My camber is now -0.9*. I think my caster is closer to 4* now though.)

If you have ajustable components in the rear, make sure they do a 4-wheel alignment. I had to convince one guy to do a 4-wheel alignment on my car by insisting that he look for himself to confirm that I installed adjustable components in the rear.
