Caliper bolt sleeve froze in place?

Caliper bolt sleeve froze in place?
I'm trying to figure out how this could have happened... I was changing out my rotors last night, and everything went fine for the passenger side. When I went to change the drivers side, everything came apart fine, but when I went to put it back together, the caliper wouldn't fit over the mounting plate. The sleeve that the upper 13mm bolt threads into, had somehow come out from the housing about 1/4", and was frozen in place. I tried everything to get it to slide back in, but it wasn't going to move.
What could've cause this to happen? The sleeve for the bottom mounting point was fine, but I need to replace the whole caliper (edit: caliper mounting plate) assy now, because of this.

Excessive heat, incorrect grease for heat range temp or bent guide pin.
Take a vise-grip locking pliars and pull it out with a twisting motion, inspect and decide.
