5 bolts will be fine on the bellhousing, just use the proper length high quality bolts.
A great choice is the TCI LS1 flexplate. Bolts on with no issues, bolts up to small bolt pattern (TH350) converters and is SFI approved. It is a little pricey (around $200), but no hassles. I tried the adapters and spacers from GMPP, and thought they were a little hokey, and not SFI approved. TCI worked out perfect.

All you should need since its a LQ4 is the stock 6.0L flex plate for the 4l80 and the stock spacer. The flexplate will have the larger th400 bolt pattern on it so you can drill it to match or get a th350 converter with dual bolt patterns on it.

thanks guys, already have a tci conv wich is drill dual bolt pattern. If I go with a stock flex plate can I buy the spacer from gm or someone else ? it came out of a 2500 hd so most likely there was a 4l80 in it right ? it was a 04 model. Also I need to buy one coil Should I just buy a gm or will parts house one be ok?

make sure you buy CAR bolts, NOT truck bolts.
all the truck bolts have an extra stud sticking out the other end to bolt various things onto. they are also a 15mm head, which is a pain on my car swapping trannys with solid mounts.
car bolts are 13mm head. def. want chevy bolts, they thread in soooo much easier than 10.9 hardware store bolts.
if you have a CAT dealer in your area, thats a different story. they are black oxide flanged head bolts and are a lil cheaper than GM. GM you have to order, CAT usually has them in stock.
big rant for a bolt..... but lots of useful info.
