What would cause crank pulley bolt to come loose?

Not a good idea at all. That can cause a hydrolock condition and if the bolt is torqued too much it may strip the crank threads, or give a false torque reading which means the bolt is not where it should be and then it will back out. Do not use thread locker on the crank bolt. Completely unnecessary.
