Causes of oil pump failures?
i know the early ones are bad, i'm just trying to figure out if there are any warning signs to look for.
does the o-ring rip, or does the pump lock up, or something else?
does it just quit all of a sudden, ~40psi > 0 without any warning, or does it gradually lose pressure over time?
i've done a million searches, but all i can find is that they're bad, never says specifically why they went bad.

they will suddenly fail. Gomer and I installed the heads on the car, which already had a cam in it. As he was tuning the car, things were fine. He pulled the car to his shop, shut it down and went to work on something else. When he came out to move it, he fired it up and had nothing. Good thing he was paying attention...i might not have.
