Default Crane Rocker Backed Itself Off

im not blaming anybody but me for this one, although i tightened the jamnut as much as i could and somehow it still backed off. the rocker is in relatively good shape, the retainer/valve tip/locks are not hurt. a new rocker will be ordered and a 4.5mm allen SOCKET will be use to torque down the jam nuts too ...20? ft/lbs. when i heard it clunk i felt like this

Did you do the final step in the Vinci instructions? I tightened my jam nut to 12 lb-ft and then tighten the jam nut and hex nut together 1/8 turn. My problem has always been getting them off again.
