Subscribing... Mine is doing the exact same thing except I can get the car to accelerate in 3rd but it takes >3K RPM. I don't know if 4th works since I don't dare spin it up to go fast enough to check... I did do a scan today (HP Tuners and am going to see if I can tell anything). Oh, my sig isn't updated. This is a 4L65E in my 69 Camaro with an LS3.

My car is also doing the same thing. I just installed headers and exhaust in my 2000 Z28 and I went for a drive. I was giving it some gas on the interstate on-ramp and when I got up to 80, it wouldn't shift and revved up and hit the limiter at 6-6500 (don't know exact rpms). I started to slow down and when I got back into town, it would shift through 1st and 2nd, but when I was going about 30 it wouldn't shift into 3rd, just rev up like it was in neutral. Now, the only way I can drive is by manually shifting it (so it doesn't try to go into 3rd). My dad thinks it might be an unplugged or burnt electrical wire, or an unplugged vacuum line. Could these be the problem as the others have too?
