HELP... Servo cover on A4 popping off?

HELP... Servo cover on A4 popping off? This has happened twice.I know the O ring is right and the components are on properly,Seems like something internal is banging it out.Its happened twice in the last 6 months. First time I thought it was a fluke but now its happened again. Anyone had this trouble or any Idea what causes it to happen? Thanks
