Well i didnt see the 456 gears so 22 mph in 4th is not so unreasonable.
But assuming this is a 4l80e since it has and input speed sensor .
Stall being 3000 that could be correct along with the difference in engine to input rpm. Provided the converter is not locked but unless i am missing something it looks like it is bearing in mind Im no tuner

It's a 4l60e in an 04 truck
Tranny has these upgrades with 15K miles or so on it.
Bushing Kit, Master Overhaul Kit, New OEM Style Filter, Updated SKF Input Sprag, New Washers And Bearings, Updated Low Roller Clutch, New Steel Oil Pan, Raybestos Indy Red Clutches, Reprogrammed & Updated Street & Strip Valve Body, Valve Body Spacer Plate, New Internal Wire Harness, New EPC Solenoid, New Shift Solenoids, New Speed Sensor, New 13 Van Pump Rotor, Corvette 2nd Servo, Super 4th Gear Servo, New Selector Shaft, Raybestos Pro Series 2-4 Band, Upgraded Beast Sunshell, Molded Steel & Rubber Pistons For Input Drum.
The convertor is a Yank TT3000. The 408 has only been in a week and I haven't been razzing on it to bad.
Normally 2000rpm in 4th locked I'd be doing about 60mph The tires are 30" high.
It seems like it's skipping 3rd gear but slipping in 4th

It shifts fine cold but all hell breakes loose when it warms up. Would the clutch just give up all in one day? It was fine yesterday and today it won't move in 4th.
