ID this intake setup? This is the intake/MAF adapter setup on my truck:
Can any of you tell me who made it? I'm about to take it off & sell it, and would like to at least let people know that much + judge a fair price

Not a cheap piece at all. On the face it should be ball milled with LS1 and say Street and Performance, Mena Arkansas on it.
S&P prices (new)
$218.00 LS1Air Cleaner 3" filter
$140.00 3pin MAF (refunded on core)
$37.00 Polished 3pin MAF adapter
$32.00 Screw in IAT
So new, it cost $427, but after you send them a MAF, you get $140 of that back. Cleaned up, you should be able to get a little bit for it.
