window tint exemption

I have been having a issue getting migraines from sunlight and have a sensitivity to headlights at night.
my eye doctor suggested i get a exemption for window tint. I can not find this application anywhere online.
if someone has had this down where did you get it?

Hmmm.... Never heard of this before.....
I wouldn't mind an exemption like that
. hehe.. Seems unusual that they would have an exemption like that instead of just encouraging you to wear sunglasses
. Curious, will they let you tint your windshield with this? You may have just uncovered a gold mine for people wanting to legally have their ride blacked out. ![Happy](https://ls1tech.com/forums/images/smilies/LS1Tech/gr_stretch.gif)
I guess alot of the police around here must have sensitive eyes too since most of their vehicles have illegal tint.

and hey, it says you need only "ONE" of the following to sign it .....
- physician licensed to practice under chapters 458, 459, or 460, Florida Statutes.
- optometrist (for sight only)
- physician who practices medicine in a military medical facility, state hospital or federal prison. The physician must include the name and address of the facility.
- An advanced registered nurse practitioner licensed under chapter 464, under teh protocol of a licensed physician.
- Physician assistant licensed under chapter 458 or 459, florida statutes.
