Default 2nd Fuel filter/Regulator blown

I am hooking up an LS1 to a custom car and I installed the OEM filter/regulator to the system. I am using a HI-PO holley intank fuel pump wich puts out over a 100 PSI all day long and we started it the first day it was done and it fired right up! we let it run for about 5 min and then the next day it would not start. We found that the pressure from the Filter/regulator was almost nothing. So we ordered a new one that was installed and it too started right up and ran a good 30 min while we test and checked other things such as water temp leaks etc.. the next day the SAME problem it would not start and the fuel pressure AGAIN was at less than 2 PSI and not the needed 58PSI. I figured the first one could have been a bad one from the start even though it was purchased brand new.... but the second brand new one going out, is a bit hard to swallow. I figure it could be one of two things. Either we have it hooked up wrong which is damaging the regulator or, the fuel pump is too powerfull, which is causing it to break. Here is how we have it connected. There is one inlet/outlet on one side and two on the other side. The one side that has just one is where we have connected to the fuel pump so fuel is going IN and the other side there is a Large outlet off to the side and a smaller outlet in the center. We have the Larger outlet on the side going to the Fuel Rail and the Smaller one in the center going to the gas tank return line.

Does anyone have a clue what the problem could be? Although I would MUCH rather keep the OEM filter and regulator my only other option is to buy a seperate pressure regulator and filter. So my second question to you all is what can I buy that I could get the correct fitting to install the stock GM push lock gas lines with? I am using rubber fuel injection hose so I would have to get some sort of push on type fittings for the hose. Also I can NOT use any regulators that mount on or near the fuel rails for many reason so it MUST be a seperate unit that I can mount close to the tank area. Any help is GREATLY appriciated.

Thanks but I have plenty of these already and I have a MasterCool kit that can make them. What I need to be able to do (If I get a seperat fuel pressure regulator) is go from whatever AN fittings to a rubber fuel line.

There should be one outlet and one return for a normal
EFI setup, maybe you have these two sides swapped?
With both ports open the one that spews fuel should be
your outlet, the return port should stay closed (or weep
only a tiny bit) until you approach setpoint pressure on
the outlet port.

But it could be that inlet and return are the two ports
that sit together, since these both would go to the
same place (tank / pump lines) and the solo is the outlet
port.

Did the reg not come with any drawings / info?
