Default my belt is coming off

ive had my belt come off my ls1 about 3 times now in the last 3-4 years, it has happend after i downshift on the freeway from 5th to 4th, is this normal or do you guys think the tensioner might be worn out already? in those 3-4 years ive probably put less then 5k miles on the car if that matters, since it was a donor motor for my 67 camaro the motor has more miles then that ofcourse, every time i replace the belt i always put a new one in so yeah any help or tips will help thanks

Its on my car because the stock one squeaked and annoyed me. Its not had any issues on my car and is a direct replacement for the stock one. Tensioners go bad all the time. I have replaced a lot of the AC tensioners on the 5.3 and 6.0 under warranty at the dealer.

What kind of car is your donor engine from? The belts and accessories are different between the F-Bodies and Corvette, for example. (They don't line up with each other and are offset.)

Throwing the belt is not normal. Do you think there is a possibility that your engine accessories are not a matched set? Can you post a picture of where the belt sits on the water pump pulley? If there isn't a regular straight grove from the belt, that could be an indication that things aren't lined up and the belt is moving around.

Hows your balancer. LS1 balancers are known to break apart and start moving around a little.
