my car had I think around 285,000 miles when i did what is in my sig. Having said that i bought my car New and know ALL the History of the car. I now have around 308,000 and it is still running good. I would do a Compression test first to determine the condition of the rings. MY thought is if the rings are ok then the bearings are ok.

Mine 5.3 Sierra has 295,000 and runs real strong in bone stock form. I bought it got it from a friend that changed the oil every 3000 and drove it easy. It probaly been driven harder the last month that Ive had it than in its entire life. It will break the tires loose @ 30. I am doing my research and plan on going with 2.5 5.3 heads and MS3 cam in the near future with the stock high milage bottom end. I agree with above (check compression). If the bottom end is getting weak any power adders will take a toll and you will hurt something sooner. Make sure you dont short cut on parts that need to be replace (ie. headbolts, GOOD gaskets, timing chain, possably oil pump, balance bolt). If your gonna do it do it right the first time. I know a guy that has 550,000 on a 6.0 truck and pulls a trailer with it 5 days a week and they drive it hard and its still running strong. Good luck

There are few different choices out there. I recommend looking into our ported LS6 oil pump. We have great success with these and have been using them for several years. We start with the LS6 pump and then port and shim it to give you a nice bump in flow and pressure.

I just posted how good my rings and cylinder walls were still good at 200k miles...and yes a new oil pump will help, before my motor was pulled from original truck it had low oil pressure and during disassembly is when I found the oil pump machine surface was wiped out...so yes an oil pump is $50-100 of cheap insurance

I'm in the same boat. Putting together a head/cam package for a 143k mile motor.
Here's what I'd do:
-All new gaskets everywhere you touch
-New lifters (GM LS7 are cheap alternatives)
-Comp Cams Trunion upgrade to stock rockers
-Melling High Volume Oil Pump and Katech/IWIS C5-R Timing Chain (need a single row with Melling - they also have a high pressure pump which is more for forged engines)
-Look at your balancer - Powerbond is a good alternative or ATI if you have the cash
-New spark plugs and wires (might as well replace)
-New fuel injectors if you plan on making above 430rwhp as insurance (FIC offers a resizing of stock injector service; Ford SVO 30lbs are also a good low-cost alternative)
-I'd do ARP bolts where there is a GM TTY bolt (head bolts, crank bolt) just because you can reuse those down the line
-Of course new valvesprings and hardened pushrods for your cam
