Depends on your goals. The 5.3 truck heads already have small combustion chambers, so easy on the milling. To go from stock 61cc to 59cc chambers for example, you would need to mill about .012". The more you mill, the more you need to worry about piston to valve clearance. With a most cam, you should not have a problem.
