Once water has been mixed in with the oil, I would atleast inspect rod and main bearings, if wear there is minimal then everything else 'should' be okay. Not all leaking head gaskets put water in the oil so I would definately get the heads checked out thoroughly for cracks. When you take the heads off, look around for a spot where they might have been leaking, i.e. broken gasket or something. If water is getting into a cylinder that piston and chamber should be cleaner than the rest.

question, should I have the heads surfaced if the pressure check comes back good?

Atleast check them or have them checked for flatness, if they need surfaced, have it done.
