So, I did call TSP and they gave me a good email address to contact Roger at yella terra. Roger sent me a replacement free of charge but then informed me that the 6645's are not meant to hold up on my TEA Trick Flow heads. Supposed to be some 6700 series in the works. I took some measurements of my heads for them to verify and maybe I'll get a discount on the 67's when they're ready.

Hopefully Yella Terra sees that picture and go back to the drawing board to fix the problem. It's sad to see a hard working guy buy a set of 500- 700 lifters and they fail!!
That's why I'm going T&D rocker arms on my big motor.... Nothing else!!

I've got a set of LS1 TFS 245s. I used shaft mount Crower Rockers on them. I have the rockers for sale on ebay if interested. You would however have to pull the heads and have them cut for the shaft mount setup.

!!! Here is the kicker, the YT6645 rocker set is for factory GM heads. Your form indicates your are using TFS 235 heads. The correct set for these heads is YT6727. Two issues are involved here.
1) The 6645 rockers are not the correct set for the TFS heads, that’s why we designed several sets specifically for the TFS heads. There are specific rocker sets for various spring pressures and other particular nuances. As I indicated the set you need is our YT6727 set. Specifically designed for the TFS 235 heads.
2) The spring pressure you are running exceeds the max allowable pressure for the YT6645 rockers especially given the high lift and serious lobe profiles you have. These rockers were designed for stock replacement or mildly modified engines and pressures between 400 & 420lbs. The YT6727 rockers will handle pressures up to 480lbs, the type of pressures engine builders typically use with aftermarket heads and aggressive camshafts.
Funny thing is, my buddy has the same 235cc TFS built by TEA as mine with a bigger cam and he has been running the same rockers for 8 years.

You are not alone...
Pretty similar situation, TFS 235 heads, been running this setup since Feb 2010. Just broke this one and I believe I just broke either the replacement or another one 22 miles later, but haven't had it torn apart to check.

Why are you guys buying Australian made parts when you have so many quality parts to choose from made right there in your good old US of A.
We make virtually nothing of any good value here, and anything that we can and do properly, our government shuts it down or attemps to. Like the modified car scene.
I just imported T&D's for my LS7 heads.. they're a work of art!!!
I do still use Rollmaster double billet timing chains. They are still quality, so far.

I think when most of these builds were done, there was no other option for the TFS heads. Meanwhile Tony is thinking..."You bought a Mamofied intake, why didn't you buy Mamofied AFR heads?" Touché Tony, touché.
