The pilot bearing supports the transmission input shaft and keeps it perfectly centered. Without it the shaft would 'droop' down just enough to cause a state of imbalance and would wear out the input shafts main bearing.

Also note that some cars don't even have pilot bearings, such as Hondas. Hence why they often chew up input shaft bearings.
