According to all of the replies, I'll just replace the internals with stock
parts.
Once I get the transmission out, I will take some pictures of the damage
and upload them.
Seems like I'll be rebuilding the entire 1-2 assembly again.

First of all, thanks to everyone that sent PM's with T56 transmissions for sale.
I decided to remove the transmission and get it fixed with stock parts as
suggest by a few members.
By the middle of next week I should have some more information regarding the
failed parts, and hopefully a good explanation of why the 1st/2nd assembly
continues to break down.
While the transmission is out I'll be redoing the exhaust and tuning it properly.
The mess that came with the car was pretty restrictive. The gas had to find
its way through two cats. (left and right cat + catted y-pipe),
a bunch of nasty bends, an expansion chamber, and finally a muffler.
Here are some pictures for your viewing pleasure. Nothing special really, just
wanted to share and keep those interested updated:
There were also pieces found inside the pipe which broke off from the right side converter:

The back side of the exhaust showing the home-made cap which will convert to a blocked off tip for a more finished look:
If I can grab another tenth from exhaust tuning I'll be happy. From there,
I'm going to try DR's and hope for a high to mid 11 pass.

The results are in!
It turns out, one of the hubs (sliders) is dented:
Some teeth are shaved off a gear (near blue dot), along with heat discolouration:
1st/2nd synchro assembly is toast. As you can see from the comparison,
one of the rings has completely lost all of the teeth!

That dented hub is Reverse, not 1-2 ???
What caused the 5-6 driven cluster to heat up? The chipped tooth isn't good, but it usually won't cause problems.. Were the drive gears heated?
The worn blocker rings on 1-2 are effects, not causes, since you've recently rebuilt. You still need to be looking for what's causing your issues.

Yes, that's the reverse hub. I suspect some debris got jammed in that
area and caused the dent; not sure what else may have caused it.
The person who inspected the 1-2 syncrho mentioned that the slider (sleeve)
would not fully engage and he suspects one the parts previously installed
was the incorrect type. I'm not totally certain as the assembly has not been
dismantled.
No clue about 5-6 heating up? It seems odd that the pattern does not
wrap around the entire set of teeth, or collar. The shop may have used
heat to remove it from the shaft?

After about 80 days of waiting, and running around to transmission shops
I finally gave up and decided to buy used.
Blake from Flordia saved me from parking the car until next season due to
back-ordered parts.
Here are a few tips for those thinking abuot rebuilding their T-56:
- Wait until the season is over. Apparently T-56 parts are rare, and on
back order.
- Find a shop that has a transmission tech in-house and knows how to
rebuild your tranny.
- Ask whether parts are available before you drop off the transmission
(this goes well with the previous line)
I finally gave up and decided to buy used.

The 1-2 slider (synchro assembly) should have been replaced to begin with. There is no other reason that it should keep breaking like that. The 5-6 driven gear in that pic is trashed. It is all discolored from excessive heat. It was either ran low on fluid or some dummy heated it up in order to remove it.

The entire 1-2 assembly was replaced along with bearings, brass blockers,
billet inserts, stronger 1-2 shift fork, 1-2 shift pads (brass?). bearings and gears (not driven gears).
The 5-6 driven gear has discolouration in one area which leads me to believe
it was heated for ease of removal.
The first rebuild was performed by a transmission shop (popular chain) who
didn't seem to bark at rebuilding the T-56.
The second tear down was taken to another shop for a quote who admitted
they never touched a T-56, but I had a good referral and also researched them
a bit before going there.
In the end the transmission was taken to yet another shop (In pieces) for
a faster turn-around and expertise with the unit. That faster turn-around
ended up being 6 weeks (long story )... so I gave up and bought a used transmission.

Wow man! You had a string of bad luck there. I would have had it rebuilt and sent back to you in just a couple days.
Are you going to be selling your pile of T56 parts? I might be interested.
