Default Transmission problems, do I need a rebuild?

A few thousand miles ago my transmission started acting up. It grinds bad when I downshift to third, and sometimes upshifting to third as well. It also pops out of third occasionally. Also, sometimes 2nd and 4th grind a bit. With the car idle, I can hear rattling coming from the transmission, but when I push the clutch all the way in it gets quiet. Are my syncros shot? Or could it be something with my clutch not disengaging all the way? Thanks in advance.

Anyone? I head from someone today that it could be my slave cylinder, does this sound right?

single gear issues/problems are almost always transmission/synchro problems. External issues usually affect all gears.
