Default Textrilia OZ700 clutch aftermath!

I brought my car to the strip for the first time with this set up. On my fisrt pass when I shifted into 3rd the RPMs did not fall past 6200. I was able to drive the car home but when you first took and released the clutch pedal, the clutch would make an awful chattering/ vibrating noise but when you were moving it would not make the noise any more. So I finally got the time to remove the clutch from the car. Here is what I found! This clutch had less that 600 miles on it




What would cause the clutch pucks to look like that?
Could it be crappy manurfacturing, a crappy driver, or crappy install?
Would that cause the chattering noise when releasing the clutch pedal?
Sorry for the questions. Just trying to figure out what is going on so it does not happen again. That clutch was suppose to handle 800 ft lbs tq according to thier web site and I know I am not near 800 ft lbs tq.

Wow, everything is toast. Did you measure for the slave distance when you put it in?
