what would cause a rattle sound at idle?

what would cause a rattle sound at idle? im getting a rattle type sound at idle and sometimes while in gear.. its not loud but anoying, i heard some one say they if there is a Twin Disk clutch that they tend to rattle... is that true?
also what would cause 4th gear to pop out or if shifting into 4th gear with RPM's over 4000, it wants to clunk going in...
help help help...

4th gear problem may be a bent fork. Mine had the same noise at idle, I think its the pump inside he trnny. Did it with a 96K mile tranny, and makes the same noise with a 1000 mile tranny.
