Flares are, well flares. Used for heat seekers.
Chaff is comprised of thin strips of aluminum that dispeses in a cloud to distract radar guided missiles.

You pump flares while you bank into a hard turn, causing ultra hot spots flying across the sky. At the very moment you pump flares you come out of the throttle to reduce the hot gases flowing out the gates and go to max g turn in the opposite direction of the missles turn, again trying to get the missle to select a different hot spot and get out of its turning radius.
Heat-seekers are much smaller and have tighter turning radius', however, they are usually used close in because the missle has to be close enough to lock onto the exhaust gates before you fire it. Its also a well known tactic to buzz the enemy with gunfire, even if its off target, while you are firing missles to give him some tracers to think about. Only the very disciplined pilots can fly, manage their radars and radios, evade gun-fire, listen for missle tones, and react by pumping flares in a few tenths of a second.
IR missles are deadly at very close range and reduce in lethality the further out.
Radar guided missles are deadly at long range, however don't have the speed to react at close in targets. The software assists the missle by creating a projected angle of attack.. Uses the targets speed and direction to give the missle an area of aim to generalize where to fly and then closes in from there.
