You can prevent that by not lifting at the jacking puck but it can be caused by anything putting stress on it or hitting the tab where the fender bolts to. I've had it happen to my passengers side from my car being low and slamming on a slow bump stop or whatever those things are called.

You'll need to take out the two bolts on the bottom of the fender. Then grab a small hammer and put enough force into the blows so that you'll hit the tab back into place. Hit it enough so that the fender pulls tight again and screw the bolts back into place.

my pass side is like this, but I am afraid to try and bend the tab back as I can see a crack in the tab, looks like it would just break off if I tried to bend it.

I had the same problem from the previous owner. Gently tap the tab with a rubber mallet. It doesn't take much force to bend back at all.
