Default Ball joints, replace just two or all four?

I am going to have my lca bushings changed so I figured I should change the lbj(s) also (car has 103k). Do I need to change the uppers also?

I would only change them if they are worn out.

if your basically rebuilding the lower arms then I would just for the sake of doing it, at that mileage it couldn't hurt and would save you the trouble of having to go back in there later to do them.

Agreed, I'm definitely going to do the lower ball joints my question is should I automatically do the upper ball joints?

The upper ball joints are easy to do if you have a dremel tool with a few reinforced cut-off wheels, so I'd go ahead and do them at the sometime, if you can.
