Having the WOT shift MPH set to zero can cause this to happen. The MPH needs to be set for a reasonable value. For instance, if your car shifts from 1>2 at 45mph, set the MPH to 35-40. If it shifts 2>3 at 75mph, set the MPH at 65-70. This allows enough room for you to set the actual shift points using the WOT RPM table. Both the MPH AND RPM table values must be exceeded before the shift will occur.
