Code scanner not working??

Code scanner not working?? I have a innova 3030 code scanner I'm trying to use on my newly fired up engine in my 71 camaro. Trying to figure out what's going on.
It's a 5.3 with a 2004 GTO / express Van tune with the proper adjustments made for the engine and my application done by Brendan at lt1swap.com
What's happening is that's its trying to read the PCM, and throws up an error. I tried this on my stock 2006 GMC truck with a 4.8 and it worked fine.

Here's my plug, I double checked Pin # 58 on the PCM plug for serial data for continuity to Pin # 2 on the OBD II plug and it's fine
I also checked the grounds and constant 12 volts on the OBD II plug and they are good too.
Engine fires up and idles fine, nothing weird seems to be happening. My check engine light is on and I wanted to pull the codes to see what it could be.
My question is, could the scanner not be compatible with my set-up because I am using minimum wires at the OBD II port? Doesn't the stock set-up have some added wires on that port that this scanner needs to access?
Maybe someone else has come across this issue? Thanks in advance

Does the ground go back to the PCM or does it go to the chassis? It may be unstable (compared to the PCM) and need to go through the harness to the PCM.
You're sure the data wire goes to Blue 58 and it's not shorted anywhere? Do the PCM and OBD2 port have 12V power key on, engine off?

I'm pretty sure the #58 wires not grounded...but I will def check that
I'll double check the PCM power, I double checked everything before hooking it all up...but you never know. I could have it on switched power..but it shouldn't be
OBD 2 port has power with engine off..constant 12 volts
I personally do not like jumpering the two grounds at the DLC port. I would recommend splitting them.
The grounds at the DLC port do not have to go back to the PCM or the engine harness. Split the grounds, and make sure you have them grounded to a GOOD, bare ground location.

I bought a cheapy OBDII scanner off eBay for $15 shipped just to make sure my old one isn't "too" old to read.
Guess what? The cheap one work lol

Any tips or areas to check would be great

The code P0155 has me concerned because that's the rear o2 sensors isn't it? and I was supposed to have that removed from the PCM programming
Code P0135 is the main 02 sensor in the exhaust manifold....what should I check on that??
P0200 Injector circuit is open? What could that be?

Bank 1 = Left / Driver Side
Bank 2 = Right / Passenger Side.
Both are front / Pre Cat O2 Sensors, so you should be ok. Bank 1/2 Sensor 2 would be the rear / post cat sensors.
Those P0135 / P0155 will not hurt the normal driving. Just on startup/warm up. Could be bad wiring to the O2 or bad O2's. Are the O2 used or new.
P0200 could be a bad injector or wiring is open or even grounded.
Who did the harness?.. seem there could be issue with it.
Check the wiring of the O2's and the Injector circuit's. Good Luck with the hunt...

It's a harness I modified myself, I know around January I did some work trying to re-rout the injector and o2 wiring because it was all out of wack when I took it apart.. even though I checked everything with a find comb, and then check myself again I could still have something "off".
The o2 sensors are new
I'll start tracing wires and see if I can figure out whats going on..
