What is your exhaust like? Do you have duals or single, and what about headers? The exhaust could be holding you back a lot. Remember you can only get as much power as you can move air. You can only move air in as efficiently as you move air out.

What's the diameter on the piping? And do you have any sharp bends in it? If the exhaust isn't restrictive and the truck runs and drives fine the problem has got to be tune related. Like others have pointed out people have used the TR224 for years and made great power. It's an all around great off the shelf cam for the LS1 engine. I would expect at least 350 RWHP with it. Right now you are putting down what my 98 LS1 put down with MAC Headers, high flow cats Lid and bullet muffler untuned.

I do not know your tuner.
But I have seen cases where tuners are lazy and do not want to spend the time to properly tune cars. 1 hr usually is not going to be enough time to tune it. There are only a few guys in the country that are that savy.
Take it to another tuner and tell them you want a dyno but definitley a street tune not just a dyno tune at wot and part throttle.

I do know the tuner, I dont think that he takes the time to get all he can out of it. Just gets it close and calls it good. I picked up my car about 6.00 pm, the next day I had a few questions to ask him, somewhere in the conf he tells me that he tuned 5 more cars that night after mine. I took mine to another dyno that same week. And its really rich. They told me I could pick maybe another 20hp just by leaning it out. Of course that was going to another $350.00 bucks!
