What flex plate is this built for? What diameter is this, and is this single or triple disc?

Its for a 4l80e bolt pattern but it also has other bolt patterns on it (3 total). I used it with a TCI 399754 4l80e style SFI flexplate. Its a 245mm single disc lockup.
