generally if i can run my fingernail across the journal and it catches on anything, groove, knick,whatever i will have it polished.. i will also check for taper, if i remember correct its like .0005 taper...

if your just freshening it up with new bearings and rings theres no need to balance...
you can have a stock rotating assembly balanced and it will make a stock engine run smoother and rev better.. can help it last longer also..
