Ram Powergrip HD LT1 Clutch Brand new, purchased from another member on here as an LS1 Clutch and didn't open the box until the Tranny was on the floor... so...

Good for up to 700HP, Jegs has the lowest price I've seen, at $479 + shipping

No flywheel, Just the clutch set (clutch disc and pressure plate). I'll snap a pic or two tomorrow and post them, it's sitting out in the garage.
