I've used them before. Had good luck, then had one come off at 70mph on my '00 1500. Then I had one come off on my '69. It's kinda like playing russian roulette. I'd recommend the hub-centric adapters if you have to use them. I'm not a big fan of the spacers, but the adapters are decent.

As long as you have enough threads engaged on the lug nuts and they are torqued properly for the stud you are using there should be no issue. Harder or larger diameter studs will require more torque to stretch them so they don't loosen. Believe it or not the center of the hub is only used to center the wheel for installation. All of the load goes thru the studs. I didn't believe this, but after talking to tire and wheel engineers from GM I believe it.
Spacing the center of your wheel outboard does increase the moment and load on the vehicle so there is some risk inherent in that.

I have some huncentric ones on my car, they are small though(1/4") Anything bigger than that and I think it would be unsafe to use with factory lugs/studs. Really with any small spacers you hardly notice the difference and with any big spacer the safety factory outways it's use.
